I, the undersigned parent or legal guardian of the minor child named below (“Child”), hereby grant permission to Wild Wonders, located in Verrado, Buckeye, Arizona, to photograph my Child during program activities. I further authorize Wild Wonders to use, reproduce, publish, and distribute such photographs for the purposes of publication and media features, subject to the following conditions:
1. Identification
My Child’s name will not be used in connection with any photograph.
Identification will be limited to stating that the child is a participant of Wild Wonders in Verrado, Buckeye, Arizona.
2. No Compensation
I acknowledge and agree that neither I, my Child, nor Wild Wonders will receive any financial compensation for the use of these photographs.
3. No Guarantee of Use
I understand that granting permission does not guarantee that my Child’s photographs will be selected for publication or use.
4. Review of Images
I understand that any photographs of my Child considered for publication will be shared with me prior to their release.
5. Duration of Consent
This consent will remain in effect unless and until I revoke it in writing. I understand that revocation will not affect any photographs already published or in use prior to the date of revocation.
This signed release form will remain on file with Wild Wonders for record keeping purposes.
By signing below, I acknowledge that I have read and understood this release, and I consent to the use of photographs of my Child under the terms described above.
